چکیده

McDonnell et al. [1], is a rare colonoscopic finding, with only a few articles published in the literature. The patients in these reports have been predominantly female and the finding has been commonly associated with collagenous colitis or diversion colitis. The main proposed causative mechanism is barotrauma [1–3], and until now, the sign has been considered to be a manifestation of certain relatively benign conditions. However, as we report here, other conditions, such as chronic cholestasis in our case, may also be associated with the cat scratch sign. A 50-year-old man was referred to our clinic for evaluation of extrahepatic cholestasis. The patient had a colonoscopic examination while undergoing tests as part of routine colorectal cancer screening. During the colonoscopy we noticed bright red, linear markings in the ascending colon, similar to the cat scratch colon sign (●" Fig. 1, 2). The other colon segments were normal. Based on all the results of the investigations, the patient was diagnosed as having cholangiocellular carcinoma, which was deemed to be the cause of his extrahepatic cholestasis. Chronic cholestasis can impair vitamin absorption, particularly the lipid-soluble vitamins (A, D, E, and K). The epithelial disruption and a tendency to bleed in the colon, resulting from deficiency of vitamins A and K, respectively, may lead to a vulnerable state and cat scratch colon. Our patient’s prothrombin time was prolonged, which was correctedwith vitamin K administration. The levels of the other lipid-soluble vitamins were also below normal (data not shown). Moreover, there is a growing body of published evidence that chronic cholestasis can impair the gut’s barrier function. In the reports in the literature it has been observed that obstructive jaundice in humans generates huge intestinal oxidative stress, which may disrupt the intestinal barrier function [4,5]. Therefore we think that chronic cholestasis and its complications may also be one of the conditions responsible for the cat scratch colon sign.

"Cat Scratch Colon" in a Patient with Ischemic Colitis

"Cat scratch colon" is a gross finding characterized by hemorrhagic mucosal scratches on colonoscopy. It is usually associated with a normal colon and is rarely associated with collagenous colitis. In a previous report, cat scratch colon was noted in the cecum and ascending colon, but has also been observed in the distal transverse colon. The patient in this study was also diagnosed with ischem...

Cat scratch colon during tumor observation prior to endoscopic treatment.

A 70-year-old woman was referred to our hospital for treatment of a laterally-spreading tumor (LST) of 40mm diameter, located in the ascending colon. The woman also had chronic renal failure. Before endoscopic treatment, magnifying endoscopy and endoscopic ultrasound (EUS) was scheduled to allow a more precise diagnosis. Initially, chromoendoscopy with indigocarmine, magnifying endoscopy with c...

Cat scratch esophagus.

Cat scratch lesion of the gastrointestinal tract is a rare entity. It was first reported in the colon, in 2007 [1]. A few case reports of cat scratch colon were published in the English literature following the initial description of this entity [2–4]. Recently, a case was described of linear esophageal tears in a man, similar to the colonic cat scratch lesion [5]. We report the second case of ...

Severe intrahepatic cholestasis in an elderly patient with primary amyloidosis and colon adenocarcinoma.

Hepatic involvement in primary amyloidosis (AL type) is not rare but is often clinically silent. However, presentation with jaundice in AL-type amyloidosis is rare, with an incidence of less than 5% reported in the literature. It is considered to be a preterminal sign. We herein report on a case of primary hepatic amyloidosis presenting with severe intrahepatic cholestasis. Viral, drug, alcohol...

"Cat scratch" colon in a female patient after a difficult screening colonoscopy.

Figure 1. Figure 1A shows cecal findings from an asymptomatic 59-year-old woman who presented for an initial screening colonoscopy. The remainder of the colonic mucosa was normal. Cecal intubation was achieved in 9 minutes and required repeated loop reduction and abdominal pressure. This image depicts the linear hemorrhagic mucosa found in patients with barotrauma, or “cat scratch” colon. Figur...
